Enrollment Procedure

For Face-to-Face Enrollment:

• 1. Proceed to the Cashier's office to pay the enrollment fee.
• 2. Present the receipt of payment to the Registrar's office.
• 3. For irregular students, consult with the Academic Dean regarding Subject Loads and Sequence.
• 4. Completely fill up the Enrollment Form.
• 5. Submit the filled-up enrollment form to the Registrar's office.
• 6. Wait for your Subject Load and Sequence to be given.
• 7. Congratulations! You are now officially enrolled.

For Online Enrollment:

• 1. Pay the enrollment fee at the Cashier's office.
• 2. Fill up the Enrollment Form completely through this link - Online Enrollment Form
• 3. Present the receipt of payment to the Registrar's office.
• 7. Congratulations! You are now officially enrolled.

Enrollment Requirements

For Incoming Grade 11 and Pre-College Students:

• 1. Original copy of Report Card showing the completion of Junior High School
• 2. Original copy of Certificate of Good Moral Character from the previous school attended
• 3. Original copy of NSO Live Birth Certificate

For Previously Enrolled Students:

• 1. Original copy of Certificate of Transfer Credential from the previous school attended
• 2. Original copy of Certificate of Good Moral Character from the previous school attended
• 3. Original copy of NSO Live Birth Certificate

Note: All documents must be submitted in person to the Registrar’s Office upon enrollment. Incomplete requirements may result in delay or cancellation of enrollment.
